If your plan reimburses for out-of-network care, reimbursement is determined by your insurance after the claim is processed. Once your account is set up, I will charge sessions through Thrizer moving forward. Depending on your eligibility, you may have different payment options:- OON Pay: You pay the full session fee upfront, and Thrizer submits the out-of-network claim. Any reimbursement goes back to you.
- Thrizer Pay: If eligible, you pay an estimated upfront amount, and Thrizer advances the rest of the session fee.
- Self-Pay: You pay the full session fee, and no insurance claim is submitted.
Consent form language
By signing this form, you authorize Thrizer to support payment processing and out-of-network billing processes related to services rendered, including benefit checks and claim submissions when applicable. Charges processed through Thrizer appear on your bank or credit card statement as Thrizer.
