Your practice chooses what clients pay for out-of-network (OON) billing. In Services, you can set a default rate for each clinician or a different rate for an individual client. The client’s saved rate takes priority over that clinician’s default. These client rates apply to OON Billing charges submitted from November 2, 2026. Changing them does not change the fees your practice pays Thrizer. See Pricing overview for Thrizer’s fees and payout examples. Before changing a client’s rate, let them know the new amount and when it takes effect. See sample client messages.

Choose which rate to change

There are two separate OON billing Services: You can keep the default or choose any rate of $0 or more. Setting $0 waives that service charge for the client. Each rate is separate: changing the Check or Bank rate does not change the Instant rate. These are non-billable service charges. They are added to the client’s payment and are not billed to insurance as part of the therapy Service.

Set your catalog rates

Use the catalog to set a clinician’s default prices. To change the rate for just one client, use the next section instead.
  1. Open Clinician Portal → Settings and select Services.
  2. Find OON Billing – Check or Bank or OON Billing – Instant.
  3. Open the row menu and choose Edit.
  4. Enter the amount in Rate ($), then select Save Changes.
Only the rate can be edited. The Service name is fixed, and these two Services cannot be deleted. Changing a catalog rate affects clients who use that default. Clients with a customized rate keep their saved rate.

Customize or waive a fee for one client

The client must have an accepted connection with the clinician, and your account must have permission to manage the client’s Services. If you see View instead of Manage, the Services are read-only.
  1. Open Clinician Portal → Clients and select the client.
  2. On Overview, find Services and select Manage.
  3. Find the relevant OON billing Service, open its row menu, and select Customize terms.
  4. Enter Rate ($). The catalog rate appears underneath for reference.
  5. Select Save Changes.
To waive only the client’s OON billing fee, set its rate to $0. The session charge remains. Set both OON rates to $0 if you want to waive the fee whether the client uses Check, Bank, or Instant. Your practice’s applicable Thrizer fees still apply. A rate that differs from the catalog is marked Customized. To use the catalog rate again, choose Reset to catalog terms from the row menu. These Services cannot be hidden or made the client’s Primary Service. The saved rate applies only when this clinician charges this client. If the client moves to another clinician, review the new clinician’s rates before the next charge; the old clinician’s customized terms do not carry over.

Review the fee when adding a charge

From November 2, 2026, under standard pricing, the matching OON billing Service is added automatically when you create an OON Billing charge. If your practice has different agreed pricing, the fee line may be absent. Review the charge confirmation and contact Support through your Thrizer account if you need to confirm your pricing.
  1. Open Clinician Portal → Clients, select the client, and choose Add Charge. If you bill for a teammate, check the selected clinician.
  2. Select the session Service. The two OON billing Services are not options in this list.
  3. Choose OON Billing and review the Reimbursement Method.
  4. Check the matching fee and rate in the confirmation. The client’s customized rate applies when one exists for the selected clinician; otherwise that clinician’s catalog rate applies.
  5. Review Client Charged and Net Proceeds before confirming.
The fee cannot be edited or removed inside the charge. The note reads, “Set in this client’s services or your catalog.” If the amount needs to change, update the client’s terms or the catalog before charging. A $0 rate still appears as $0.00. Switching between Check or Bank and Instant changes the fee to the matching Service and rate. Self-Pay charges, including no-shows, miscellaneous charges, and non-billable Services, do not include an OON billing fee. Instant Reimbursement requires an available insurance estimate and the other eligibility requirements. If Instant is unavailable, review the available Check or Bank option and its fee before confirming.

Understand the confirmation

  • Client charge breakdown shows the session, the applicable OON billing Service, and Client Charged: the total the client pays. For Instant, the insurance estimate also affects that total.
  • Clinician breakdown shows the applicable fees your practice pays Thrizer and Net Proceeds: the amount remaining for your practice after those fees.
Your client-facing rate and Thrizer’s fee are separate amounts. Waiving or lowering the client’s OON service rate changes the client’s payment and your payout; it does not waive or lower Thrizer’s fee.

Manage fees for a team

Practice administrators and team owners can open Clinician Portal → My Team, open the teammate’s row actions, select View Profile, and open the Services tab to edit either rate. Check whose catalog you are editing; the note refers to that teammate’s clients. Staff with permission to manage a client’s Services can customize that client’s rate. They cannot edit a teammate’s catalog. When charging for another clinician, review that clinician’s catalog rate. A customized client rate applies only if it was saved for that clinician and client.
