Before you start
Use this guide when BCBS of Alabama is a required Local / Host Plan for your practice or claim-service location. Your practice coordinates registration and any required clinician records so the payer can process affected claims. Registration is separate from joining a network and does not guarantee reimbursement. Not sure which plan applies? First identify your required Local / Host Plan. Open the Medical Practitioners page. Under the instructions for practitioners who are not interested in participating, download the Auxiliary Provider Application. Do not use the separate network-participation or Lucet application.Registration steps

Choose the request
For a new record, select Add New Provider/Facility. If the clinician already has a record, confirm the appropriate update request with Alabama instead.

Identify the clinician and practice
Complete Provider Information with the clinician’s individual NPI, license, specialty, and requested service-location information. In Payee/Remittance Information, enter the actual practice legal name, TIN, and applicable payee NPI.

Keep reimbursement going to the client
Under Please make all payments to, select To Patient for BCBS claims through Thrizer. The payee-information fields do not mean you should elect payment to the clinician.

Review, sign, and fax
Have the authorized person review and sign the application, then fax it to 205-220-9545, as shown on the current form. Keep a copy and ask Alabama to confirm when the record is active.
