Before you start

Use this guide for a required Anthem Local / Host Plan in Colorado, Connecticut, Georgia, Indiana, Kentucky, Maine, Missouri, Nevada, New Hampshire, New York, Ohio, Virginia, or Wisconsin. These markets document the same initial non-participating registration path. Other Blue companies in those states may use different processes. If registration is incomplete, claims can be delayed. Not sure which plan applies? First identify your required Local / Host Plan. Registration is separate from joining a network. For California, use the separate Anthem Blue Cross California guide and complete the required Blue Shield of California registration too.

Register through Availity Essentials

1

Sign in with access to your practice

Sign in to Availity Essentials. If your practice is new to Availity, create its organization account and designate an administrator. Ask that administrator to check your permissions if needed. An Availity account is access to the tool, not registration with Anthem.
2

Open Anthem's enrollment application

Select the applicable market and Payer Spaces, then Anthem. Under Applications, open Provider Enrollment and Network Management. Follow your market’s current official instructions if the application label has changed.
3

Choose the out-of-network option

Select Out-of-Network / Single Case Agreements. This shared label does not mean every registration requires a single-case agreement or an in-network contract.
4

Register the practice and include its clinicians

Complete the required organization and individual-clinician information. Your practice coordinates the records and their associations; clinicians in a group are not establishing separate practices.Do not assume a bulk clinician-roster upload is available. The state articles do not establish that capability, and most say it is still in development. If the application does not let you associate your clinicians, use Chat with Payer in the Anthem payer space for the required steps.
5

Review and confirm the records

Review the application and documents before submitting. Keep the confirmation and ask Anthem when the practice and required clinician records are active. Do not use network participation as a substitute if the out-of-network choice is missing.

Updating an existing record

The linked state articles other than Ohio direct demographic changes to My Providers → Provider Data Management. Ohio’s updated article does not provide those maintenance instructions; ask Anthem for the current Ohio update process. Avoid submitting a new initial registration just to change an existing record.

Official instructions for your state

Use the article for the market where your practice needs to register: Colorado, Connecticut, Georgia, Indiana, Kentucky, Maine, Missouri, Nevada, New Hampshire, New York, Ohio, Virginia, Wisconsin. If a screen differs, confirm the organization permissions and payer space with your Availity administrator. Then contact Anthem through Chat with Payer or its provider-support channel.

What happens next

Once each required plan confirms registration and the required clinician associations are active, notify Thrizer support and review what happens next. The payer controls registration and claim decisions. Registration does not guarantee reimbursement or recognition of every clinician category. For associate, pre-licensed, or supervised clinicians, ask the payer about recognition and review Thrizer’s supported claim structure.
