Before you start
Use this guide when Louisiana Blue is a required Local / Host Plan for your practice or claim-service location. Choose a professional non-participating record application so the payer can establish a claims-processing record without a request to join its network. If registration is incomplete, claims can be delayed. Not sure which plan applies? First identify your required Local / Host Plan. Open the current professional-provider page
Go to Louisiana Blue’s Professional Providers page and open Join Our Networks. Despite its name, this tab also contains the two Professional Non-Participating Record Application links. Opening the tab does not apply for network participation.

Choose with or without CAQH
Select Applying with CAQH or Applying without CAQH for your situation. Do not choose either Professional CAQH Credentialing Packet link, which is for participation or joining an existing network group.

Identify the actual signers
DocuSign first asks for the Authorized Representative and Provider names and emails. Enter the correct people when ready to begin: starting the signing process can send emails to those signers.Your practice coordinates its billing information and clinician records. If you need to establish a new group or associate clinicians and the packet does not explain how, ask Louisiana Blue which record or affiliation request it requires.

Complete the non-participating checklist
Follow the packet’s record-only checklist and supply the requested documents. Review everything before signing and submitting through DocuSign. Do not sign a network contract as a substitute for non-participating registration.
